* Credit cards: If you would like a credit card you must contact Creative Benefits Directly at http://www.creativebenefits.com. Credit cards will not automatically be sent to you. There will be a set up fee and an additional charge for the credit card. The total will be deducted by Creative Benefits.
* If you loose the card, please contact Creative Benefits directly at dthomas@creativebenefits.com or Debbie Thomas (760-509-0139).
* Please remember, receipts must accompany all claim forms.
* Reimbursement can be either mailed to you personally or directly deposited into you bank account.
