Full time employees have the opportunity to sign up for the Avesis Vision Care Plan.
Changes in the Plan
There are no changes to the plan coverage.
Due to security reasons, Avesis will be issuing new cards. New cards will no longer have social security numbers listed on the front of the cards.
Rates have been increased by 10%. Rates for the 2005-2006 fiscal year are:
* Employee only $6.59 per month
* Employee plus one 11.54 per month
* Employee plus family 17.14 per month
