Outstanding Prerequisites
Don't relax too much now that you've been accepted to pharmacy school! Remember that you must successfully complete all outstanding prerequisites with a grade of C or better AND maintain your GPA at or above the 2.5 minimum. All prerequisites must be completed before August 15, 2010. Transcripts from the spring semester and summer sessions are due on June 15, July 15, and August 15, respectively.
Students completing coursework during the summer must provide proof of registration from the institution(s) at which the courses will be completed. Proof of registration must include the institution name, course title (course number and name), credit hours, and dates of the summer session. Proof of registration for coursework to be completed during the summer may be submitted via email, fax, or postal mail and is due on June 15.

Complete the FAFSA
If you were unable to submit the FAFSA by the March 1 priority date, you can still submit the completed form as soon as possible. You need to do so in order to be considered for financial aid assistance. The website www.fafsa.ed.gov will guide you through the application process.

Criminal Background Check and Drug Screen
PCSP contracts with the company, Certiphi Screening, to administer criminal background checks and drug screens. Admission to the School of Pharmacy is conditional pending the results of the background check. The criminal background check and drug screen must be completed by July 1, 2010.
Information regarding completion of the criminal background check and drug screen will be emailed to you (@presby.edu address) on May 1, 2010.

Online Submission of Required Information
The required information listed below must be submitted online via E*value by July 1, 2010. Forms will be available for download from E*value. Login information for E*value will be emailed to you (@presby.edu address) on May 15.
Proof of Health Insurance
Students enrolled in the School of Pharmacy are required to have and demonstrate proof of health insurance; you are responsible for the costs of their own health insurance. Documentation of proof of health insurance must be submitted by July 1, 2010. Any student not providing proof of health insurance will be automatically enrolled in the PC-sponsored student health insurance plan. Immunizations
Pharmacy students at Presbyterian College are required to provide documentation of completion of the immunization guidelines. Immunization records must be submitted by July 1, 2010. You are required to show proof of immunity (titers) or vaccination records and incur all costs for the following vaccinations:
- MMR (measles / mumps / rubella)
- Polio (oral or injection)
- PPD (tuberculosis; patch test not accepted)
- Diphtheria / pertussis / tetanus (vaccine within the last 10 years)
- Varicella (chicken pox)
- Hepatitis B (3 shot series; at least documentation of 2 shots is required by July 1 of the year of enrollment)

Required Medical Clearance
All PCSP students must provide emergency contact information via completion of a medical clearance form. The medical clearance form must be submitted by July 1, 2010. If you have a medical condition that requires special learning considerations, you must also submit documentation from your physician regarding your medical condition.

Contact Information
The School of Pharmacy requires students to provide current contact information, including a mobile telephone number, while in the professional program. You must also provide the School with emergency contact information. Contact information must be entered into E*value during orientation. Changes should be made to the electronic record as needed to keep information up-to-date.

FERPA Acknowledgment Form
The Family Educational Rights and Privacy Act (FERPA) designates certain information about students as "directory information" and gives the College the right to disclose this information. Under the provision of FERPA, students have the right to withhold the disclosure of this information.
Academic Accommodations Request
If you have a disability that affects academic performance, you must register with the School of Pharmacy Office of Academic Affairs, using the Request for Disability Support Services form (available online via E*value). The Office of Academic Affairs will assist you in submitting the required documentation and in identifying those accommodations that will benefit you as a student. The initial request for academic accommodations for a disability is due July 1, 2010. Please be aware that renewal of accommodations is not automatic and requires student initiation each semester.
Vehicle Registration
All persons who shall regularly or occasionally drive, operate or control, park, or let stand, or otherwise use or maintain a motor vehicle of any type on the land or property of Presbyterian College is required to register the vehicle within 24 hours after the vehicle is brought to campus. Emergency Alert System Registration
