Instructions for Obtaining Enrollment Verification on Students for Insurance Purposes
Current students no longer need to bring health insurance forms to the Registrar's Office nor mail the forms to the office. Students may now request enrollment verification online and get an immediate response. Enrollment verifications are now processed through the National Student Clearinghouse. Please share this information with parents. Please follow the instructions below to obtain enrollment verification.
Go to BannerWeb. Enter the student's User ID and PIN. Click on Login. Select Student, then Student Records. Scroll down and click on National Student Clearinghouse. Select enrollment for ALL semesters. The form will appear on the screen. Print it and mail or fax to the requestor. The form may be brought by the Registrar's Office to have the college's stamp and seal placed on the form.
